Dear Parent/Guardian,

We are pleased your son/daughter has chosen to participate in athletics at Oxford College of Emory University. While it is true that participation in athletics will add greatly to the collegiate experience, there are inherent injury risks in athletic participation. So we at Oxford College may better serve your child, please do as follows:

   1. Please download the following forms, complete all of the information requested and return via mail.

   2. We need original signatures so please do not fax or e-mail these forms.
   3. All information requested is due no later than August 1st of the student's incoming year.
